Mons pubis is the fatty tissue covering the pubic bone. Its main function is to prevent pain during sexual intercourse, by forming a cushion over the pubic bone. This area also contains glands which secrete pheromones for sexual attraction.

What Causes Enlarged Mons Pubis?

Monsplasty is also known as pubic lift, puboplasty or venus hill surgery. There is a triangular area of hair-covered fat tissue in the bikini region which may become saggy and loose due to several reasons.

The following may cause an enlargement in mons pubis:

• Weight gain,
• Child birth, especially by C-Section,
• Sudden loss of massive weight after bariatric surgery,
• Aging,
• Genetic reasons.

How can the mons pubis be reduced in size?

There are two main surgical options to reduce the size of mons pubis:

• Monsplasty,
• Mons pubis liposuction.

Mons pubis may be reduced in size by choosing the appropriate method for each patient. Monsplasty is the preffered method in patients with excessive loose skin that needs to be removed surgically. It may be combined to an abdominoplasty (tummy tuck surgery). Because during an abdominoplasty, excessive loose skin and underlying fat tissue is removed from the abdominal area and the remaining enlarged, saggy mons pubis may become more prominant.

Mons pubis liposuction can be an alternative in patients without excessive loose skin. Fat tissue can be homogenously suctioned using plastic cannules. The fat tissue removed by liposuction can be used as filler in other areas of the body such as, labia majora (labial puff) or the G spot in the vagina (G shot).

Does a tummy tuck lift the mons pubis?

Abdominoplasty (tummy tuck) procedure may remove excess fat and tissue from the abdominal area and so a tummy tuck does lift the mons pubis, but may not be enough for all patients. For a more balanced look, monsplasty can be combined to an abdominoplasty to remove all excessive skin and underlying fat tissue in the area, leading to a lean belly and pubic area.

How do I know if I need a pubic lift?

If you are overweight, you are recommended to go on a diet and exercise programme to reach a healthy weight before surgery. Losing more weight after surgery may necessitate additional surgery to remove excessive loose skin tissue.

If you have diabetes, it should be regulated before surgery for better wound healing.

Chronic health problems like hypertension, heart disease, thyroid disease should be consulted and stabilised before surgery to decrease surgical risks.

Before monsplasty

• Arrange someone to drive and accompany you on the day of surgery,
• Stop consuming alcohol 2 days before surgery,
• Stop smoking 2 weeks before surgery,
• Stop taking blood thinner medicine like aspirin, heparin (after consulting your doctor) and vitamin supplements,
• Do not eat or drink anything 12 hours before surgery.

Anesthesia for monsplasty

Monsplasty can be performed under local anesthesia or under general anesthesia. Sometimes sedation may be combined with local anesthesia.

It is most commonly performed as an outpatient procedure, which means the patient can go home after having some rest in the clinic on the same day of surgery.

How is a monsplasty performed?

Here is what you will experience on the day of your monsplasty surgery:

• You come to the clinic or hospital fasting,
• You are evaluated by the anesthesiologist before surgery,
• General anesthesia or sedation combined with local anesthesia is applied by the anesthesiologist,
• A horizontal incision is made on the bikini region and a wedge of skin and subcutaneous tissue is removed,
• Sutures are used to tighten the muscles and subcutaneous tissues,
• Skin is closed with self dissolving esthetic suturing,
• After having some rest in the clinic or hospital, the you may go home on the same day.

How long does a mons lift take?

Pubic lift or mons pubis lift surgery usually takes about 30-60 minutes, most patients can go home on the same day of surgery after having enough rest at the clinic.

How is the monsplasty scar?

Monsplasty scar is similar to the scar of patients who have had a cesarian delivery. It usually stays under the bikini line and in most patients it is hardly visible.

After Monsplasty

Here is what you can expect after monsplasty surgery:

• It is normal to have pain and pain killers other than aspirin can be used,
• First 48 hours, the wound will be protected by pressure applying bandages,
• After 48 hours, you can take a shower and is advised to keep the wound dry and clean just by spraying water,
• It is normal to have swelling and bruising for the first 2-3 weeks,
• You can start walking as an exercise at a slow pace 1-2 days after surgery,
• You can travel and return to work one week after surgery, but should refrain from heavy lifting and straining,
• You can start exercising, having sex and swimming 6 weeks after surgery.
